The following form gathers all the information I need from you throughout the cover design process. The intention is to reduce how often you need to provide me with information, making the process more fluid for both of us.

There may be some questions that you're not able to answer just yet. Feel free to skip those. You can always update the form later.

Please do your best to fill it in as completely as possible.

IMPORTANT: Please note that I am dyslexic. Be sure to proofread any text that will appear on your book and write it exactly how you'd like it displayed. I will copy and paste it onto your cover and may not catch errors on my own.

About Your Cover Art

In the below section, you'll provide details about the cover art we're creating for your book. This will help guide me as I create your cover.
This is the deadline that you need your completed eBook cover or cover art delivered. Often, this is needed long before the paperback cover and is the main deadline that I will operate by to ensure your cover is delivered on time.

About Your Paperback Cover

In the below section, I'll gather information about your print cover. You only need to fill this portion of the form out if you plan to have me create a paperback cover for you and it was included in your package.
Please select a date that provides you with enough time on your end to order a proof and seek proof corrections, if necessary.
This must match the number of pages in your final formatted manuscript. Please note that providing the incorrect page count may result in an aditional charge if a second paperback setup is needed as a result.
Select yes if you need both Amazon KDP and Ingram Spark print covers (as an example). Standard Custom Covers include 1 setup in the package price. Premium and Elite Custom Covers include 2 setups in the package price.
I will copy and past eyour blurb exactly as it is written here. Please double-check spelling and grammar.
For example, do you want specific lines written in bold, in a different color or font? If so, please indicate that above.
I will include title and author name automatically. If you'd like anything else, please describe what you need above.

About Your Download Files

In this section, please indicate the files you need delivered when your cover is completed. Exporting is timely, please only select what you absolutely need.
Title and name images are created from the design you will approve on your final cover and all come on a transparent background.
Audiobook covers are only included upon request and must be included as part of your package or there will be an additional fee.
